How do I schedule an appointment?
You can request an appointment here via our website or you can email us. We will respond within 1 business day to your request. Please indicate the reason for your visit, your phone number and your preferred range of dates, days of week, and time of day. You can also call our office and one of our staff will get you scheduled.

What do I need to bring for my appointment?
You will need to bring your insurance card(s), a photo ID, and any medical records including x-rays, MRIs, or CT scans relating to the reason for your visit. Also, please bring a list of any medications you are currently taking and any to which you are allergic.
What if I don't have insurance?
We understand that not everyone has insurance. We do require payment at time services are rendered. We are happy to sit down with you to discuss payment options. As one of the options, our financial manager can work with you to establish a customized payment plan to suit your needs.
What forms of payment do you take?
We accept most major credit cards, ATM cards, cash, money orders, and personal checks.
Do you accept insurance?
Yes, we accept most major insurance, Medicare, and Medicaid, and we will file the claims for you. As the patient you are responsible for co-payments and fees not covered by your insurance plan. If you have more than one insurance, please be sure to provide us with information pertaining to both of your plans.
